PU-AMI’s Context & strategy:

Première Urgence - Aide Médicale Internationale (PU-AMI) is an international, non-profit, non-political, and non-religious humanitarian organization which aims to provide rapid global response to the basic needs of populations affected by humanitarian crises to help them regain independence and dignity. PU-AMI provides assistance to more than 6 million people throughout 24 countries, with more than 2,500 staff worldwide.
PU-AMI adopts an integrated approach, to identify and understand the full array of the needs of the people affected by crises. This innovative approach is at the core of every PU-AMI-supported projects, to better target and prioritize actions, stabilize and improve the situation of the most vulnerable groups. It allows the teams to take all aspects of a problem into account, to come up with an efficient combination of solutions, which will have a powerful and long-lasting impact on the affected populations. In this respect, the organization runs around 190 projects a year worldwide, most of which are focused on Health, Nutrition, Food Security, Water, Sanitation and Hygiene, Psychosocial Support, Education and Protection activities. 
In Yemen, PU-AMI is supporting 18 health facilities (Health Centers, Health Units and Therapeutic Feeding Centers) in four Districts in Hodeidah (Al Mighlaf, As Sukhnah, Al Mansuriyah and Al Qanawis), three Districts in Raymah (Bilad At Ta’am, Al Jabin and Mazhar) and two Districts in Taizz (Al Mokha and Mawza). PU-AMI’s operations are managed by two field offices in Hodeidah and Mokha cities, both supported by the Sana’a coordination office. PU-AMI recently established a second coordination office in Aden. 
PU-AMI is participating actively in clusters and sub-clusters (health; nutrition; food security and livelihoods; WaSH) and works closely with relevant government departments.

Overall objective

The HF Medical officer will assist the GHO’s Staff in implementing emergency intervention and providing full scope of primary care services, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance to all patients. Ensure and organise medical activities for patients in accordance to the context, to PU-AMI protocols and to universal hygiene standards taking into consideration quality and continuity of care and ensure supervision and training of he HF team. In addition, a HF Medical Officer  is a member of PU-AMI H&N team. In that capacity he/she are expected to be familiar with the philosophy, polices, goals and objectives of PU-AMI and GHO/DHO and work collaboratively to achieve them. He/she must be sensible of, and comply with, all Health Facility policies and procedures, as well as patient right state and MoPHP regulations.

Tasks and Responsibilities

1.    Ensure and conduct primary health care services at the health facility level;
2.    Manage the health facility pharmacy, medical supplies and equipment;
3.    Organize, manage and train the Health Facility health and nutrition team
4.    Facilitate the communication between GHO staff (OTP) and  Nutrition Supervisor;:  Report ,Request..
5.    Set up and manage an efficient referral system;
6.    Medical data collection, monitoring, analysis and reporting.

Specific Goals and Related Activities

1.    Ensure and conduct primary health care services at the health facility level:

  • Work in close cooperation with HF staff in all HF PHC services to ensure the quality of the activities;
  • Ensure support for GHO MD in conducting the medical consultations with full history taking and clinical examination;
  • Ensure support for GHO MD in Provide a diagnosis and prescribe treatment accordingly according to PU-AMI protocols and universal precautions; 
  • Provide the caregivers, clients with information related to visit schedule, dates of the subsequent visits, doses required, the potential side effects of the drug, and prevention;
  • Ensure support for GHO MD in apply the Integrated Management of Childhood Illness (IMCI) protocol for CU5 consultations;
  • Ensure support for GHO MD in manage and report any suspected communicable disease case according to PU-AMI protocols and national Disease Surveillance guidelines; 
  • Execute any action requested by the Health Facilities Activity Manager as part of the outbreak response plan; 
  • Record clearly the health status and medical prescription (anyone should be able to read it);

2-    Manage the health facility pharmacy, medical supplies and equipment:

  • Make regular monthly drugs consumption report, order and inform about needs before shortage of drugs;
  • Participate in the preparation of the necessary equipment for the daily consultations;
  • Contribute and monitor the HF pharmacy: availability of the relevant drugs and medical supplies;
  • Contribute and monitor the quality of the disinfection and sterilization process of the material and medical equipment;
  • Is in charge of new needs identification, in terms of drugs and medical equipment;
  • Collaborate with GHO MD in indicating the HF needs in medical equipment and drugs;
  • Participate in the monitoring of the waste management according to PU-AMI waste management procedures, infection control. 

3-   Ensure and organize the communication between Nutrition Supervisor   and OTP nurse 

  •      Ensure the submission of report to nutrition supervisor on time.
  •       Raise the needs of the nutrition activities to nutrition supervisor.
  •        Communicate with nutrition supervisor and / or SC TL on the referral of   complicated SAM/malnourished children to SC/TFC after medical evaluations of the child; 
  •     Ensure submission of the beneficiary list of the FBs to Nutrition supervisor and follow up of the distribution of FBs as support for Distribution assistant and collection of beneficiary list and cards after distribution to be submitted to Nutrition supervisor.
  • Ensuring support for Nutrition superviser in  Health and nutrition éducation is  disséminâtes to the Target  bénéficiaires 

4-    Organize, manage and train the HF health and nutrition staff:

  • Responsible of planning the activities of the all HF team members to cover the patients’ health needs;
  • Supervise and motivate HF health team and more particularly staff carrying out medical activities;
  • Follow and collaborate with all the staff in following the implemented activities correctly and scientifically;
  • Organize and work in collaboration with GHO MD and other HF GHO staff;
  • Participate to the organization of the HF services in collaboration with GHO MD and other HF staff;
  • Check the good course of medical activities within the HF (IMCI, vaccination, CMAM, Lab, pharmacy, FP, ANC and PNC);
  • Build the technical capacity of HF GHO staff following national and PU-AMI medical protocols;
  • Train a multidisciplinary HF team and particularly staff carrying out medical activities (formal and informal training);
  • Provide training for the staff under his/her supervision as advised by PU-AMI;
  • Strengthen link between CHVs and health facility with support from nutrition supervisor 

5-    Set up and management of an efficient referral system:

  • Organize the internal referral process of the patient (triage, IMCI, vaccination, CMAM, FP, ANC, PNC, pharmacy);
  • Manage the emergency and electives referral cases according to PU-AMI referral protocol;
  • Decide the transfer of the patients to the hospital of reference, in collaboration with the HF AM and GHO/DHO; 
  • Refer complicated SAM/malnourished children to SC/TFC after medical evaluations of the child; 
  • Refer complicated deliveries as well as complicated pregnancies (if there is no HF OBS/GYN specialist) to tertiary HF; 
  • Ensure a medical follow-up of quality and if necessary (and possible) refer to other more specialized HF;
  • Ensure the follow up of the payment of the transportation allowances of (Deliveries and  complicated SAM )and clearance of advances on time according to the SOPs of referral  

6-    Medical data collection, monitoring, analysis and reporting;

  • Collect reports from all staff members, review them, and document them;
  • Compile and send the validated reports in due time to the HF AM;
  • Ensure quality information and presentation in all reports;
  • Participate in the development of the plan of action;
  • Submit weekly and monthly health and nutrition reports of the HF services; 
  • Monitor the medical activities, analyse them in the context, with a public health approach;
  • Make propositions of possible evolution if necessary;
  • Report immediately to HF AM any problem arising in the service, especially the loss/robbery/damage of medical equipment or supplies;
  • Be aware of the security rules and social and political situation in the field at any time and provide feedback to HF AM.

In general, the HF Medical officer has to be flexible in performing his/her duties to ensure that beneficiaries are getting good quality services and perform other additional duties on request of the Health Facilities Activity Manager.
